How to refer
Step 1 - Send the order
Fax a completed order form to (800) 430-2202 with: patient demographics and a copy of the insurance card, recent office/clinical notes, relevant labs, and prior therapies. Prefer the phone? Call (800) 996-0978.
Step 2 - We verify & authorize
We verify home infusion benefits and obtain prior authorization, typically within 10–14 business days, while keeping your office informed throughout the process.
Step 3 - Care begins
Once approved, the first nursing visit is scheduled and medication is delivered 2–3 days ahead of the patient’s scheduled appointment.
